Make an Appointment

To make an appointment with Noah’s Ark you may call our office during business hours. We will also by happy to make your appointment with an on-line request. Once we receive your request, we will contact you to confirm a date and time that is as close to your request as possible. This form should only be used for requesting appointments that will take place at least one business day after the time of the submission of this form. If you have an urgent need, please call our practice directly at: 828-524-6121. Please note that all appointments do require a $79 deposit. This deposit is refundable as long as your appointment is cancelled at least 24 hours in advance.
